Document Security Levels:

Unclassified documents are identified by a grey background and can be viewed and shared with the general public without risk of breach of confidentiality or security risks.
Confidential documents are identified by a blue background and would cause "damage" to security or breach community confidentiality if released to the general public. Government employees and emergency responders may access confidential documents by completing a request form and receiving a user name and password.
Secret documents are currently not used.
Top Secret documents are identified by a yellow background and would cause "exceptionally grave damage" to security or breach individual confidentiality if released to the general public. Health department employees of the agencies listed below with emergency plans hosted here may access top secret documents by completing a request form and receiving a user name and password.

Request for clearance can be obtained by completing a request form.

*Notes: Pandemic Influenza planning has been integrated with Public Health Emergency Planning to reduce duplication and maximize impact. The template provided by DHSS is divided into Annexes (shown below) and the information in those Annexes is noted in the above table of contents.

Sunshine Law Disclosure:

Full disclosure of the Polk County Health Center Response Plan for Disasters, Public Health Emergencies, and Pandemics would impair its ability to protect the safety or health of persons and public interest in nondisclosure outweighs the public interest in disclosure of these records.
